How do I create waivers and intake forms?

Open the Forms tab and click create. The drag-and-drop builder supports text, long text, multiple choice, checkboxes, date and signature fields. Preview it, publish it, and mark it required so members must complete it before their first booking. Submissions are searchable and exportable to CSV.

The lifecycle of a form

  1. Build it in the form builder.
  2. Preview it to check the questions read clearly, the order is logical, required fields are marked, and it works on mobile.
  3. Publish it to make it available to members.
  4. Link it to bookings so it is required before a member can book.
  5. Review submissions as they arrive.

A required form blocks the first booking. That is the point, but it also means a badly worded or overlong waiver is a direct drag on new-member conversion. If sign-ups stall, your waiver is worth re-reading.
